| PURPOSE: |
DSCC works to help children with disabilities, and those who have conditions that could lead to disabilities, reach their full potential. By helping to provide appropriate specialized diagnosis and medical care, DSCC assists children to grow to the full extent of their abilities. |
| SERVICES: |
Coordinates care and authorizes payment for a broad array of specialty
services including diagnostic evaluation, outpatient specialty care,
in-patient/surgical treatment, durable medical equipment and supplies,
therapies, medications, and travel reimbursement. |
| ELIGIBILITY: |
Children 0-21, who have a medically eligible condition, are legal residents of the state of Illinois, and have a legally responsible adult who manages their care. Financial eligibility is determined upon receiving financial application. |
| INFORMATION: |
There are several categories of impairment, including orthopedic, nervous system, cardiovascular, craniofacial anomalies and other disfiguring conditions, hearing loss, organic speech impairments, cystic fibrosis, hemophilia, certain genetic and metabolic conditions, eye and urinary system impairments. |
