PURPOSE: Serves as initial access point into the Early Intervention system.

ELIGIBILITY: For children birth to three with suspected developmental delays OR a medical diagnosis of a condition that has a high probability of resulting in developmental delay.
INFORMATION: The central office for our six-county area is in Freeport.  Their phone number is (815) 297-1041.  There are Child & Family Connections offices statewide.  People outside our particular 6-county area can call (800) 323-4769 to get the number for the office that serves their area.
